Beneficiaries not covered under category-I and category-II or who are not govt. servants/pensioners or their dependent family members.
Cashless treatment with coverage up to INR 5.00 lakhs per year per family on family floater basis. In case of more than five members, remaining members enrolled as a separate unit subject to the capping of five members for each such additional unit.
The beneficiary can apply through online system directly by visiting https://www.hpsbys.in/ or through Lok Mitra Kendra/Common Service Centers for enrollment and uploading relevant documents under the scheme. The LMK/CSC will collect INR 50/- per family up to unit of five for enrollment/preparation of e-card under this vibrant scheme. Himcare documents required are:
- Aadhar card
- Permanent residence proof
- BPL certificate copy authorized by the Panchayat Sachiv officer
- Proof for registered vendor
- Registered form signed by MC/NP/NAC
- For MNREGA workers, MANREGA job card
- Proof for widow/divorcee/unmarried/single women
- Document proof for handicapped applicants
- For old age people, age proof should be mandatory
- Certificate proof for anganwadi workers
- Document proof of ASHA workers
- Certification is required in which category you have applied
- Passport size photo
This scheme is being implemented on co-payment basis. Under this scheme, differential premium rates have been decided based on the categories as follows:
- Category 1: Zero premium amount
- Category 2: INR 365/- per year
- Category 3: INR 1000/- per year
