All members of primitive tribal groups irrespective of their economic status

Families holding Mukhyamantri Mazdoor Suraksha Card and Nirman Shramik Card

At the community level - Village organization and cluster level organization

At the mission level - District and block office

All the government hospitals that provide hospitalization

Shall be an Indian citizen

Shall fall under BPL list and has Family Health card (All families which are registered in the BPL list issued by the Panchayat/urban local bodies are eligible to get the family health card)

Identity Card

Residence proof

Voter Id card

Mobile number

Passport size photograph

INR 20,000/- per family per annum

In case of serious illness the limit is INR 30,000/-

It covers only medicine and investigation of the hospitalized patients only

Physician fees and bed charges are paid by the patient
