What do you get?
Who should get this plan?

Nearly 76.6% of people with diabetes in India live with uncontrolled diabetes, as per a nationwide study, which increases your risk of developing serious complications. Taking control of your sugar levels is important for keeping complications at bay.

Your diabetes medicines need fine-tuning from time to time. Advance care plan empowers your doctor to understand and cover the gaps in a smarter way.

Sudden changes pose more risk than constant high sugar values. Round the clock monitoring may help you find the answer.

With this plan, you can restrict further worsening of your symptoms and long-term diabetes complications by understanding your body’s unique response to stress, food choices, and medicines.

Do you know around 70% of people with prediabetes eventually develop diabetes? It is important to get a screening done early and get the right advice as per your test results. Remember, borderline diabetes is completely reversible with timely management.

People with type 1 diabetes require frequent changes in therapy before a stable plan is figured out. The Advance Care Plan will highlight your body’s needs and and help you figure out how to balance food, exercise, and medicine.

The plan gives you a smart sensor that doesn’t require finger-pricking. You can get sugar readings for next 14 days at an interval of every 15 minutes with one-time application.

If you are a parent of a child with diabetes, we encourage you to try out our plan. The smart device is completely safe to be used in children above 4 years of age.

The genetic risk associated with diabetes is very complex. If the father has diabetes (type 2), the risk factor is about 30%, slightly higher if it is the mother. If both parents have diabetes, the risk factor jumps to about 70%.

While keeping physically active helps your body utilize extra sugar in the blood, sedentary lifestyle is considered a strong risk factor for developing diabetes.

Relative to overweight people, each kg of weight gained annually over 10 years increases the risk of developing diabetes in the next 10 years by 49%.

People with thyroid conditions (hypothyroidism) and polycystic ovarian syndrome (PCOS) can disrupt the fine balance of glucose metabolism, which can lead to diabetes.

As you age, risk of diabetes increases even in absence of other risk factors. It’s both important and useful for healthy people to understand how their body is processing sugar.

Why should you get continuous glucose tracking?
Looking beyond HbA1c is important

Time-in-range is a new metric that people with diabetes must track. It is important for a person with diabetes to spend >70% of day's time within the target blood sugar range (usually 70-180 mg/dL). Controlled time-in-range brings down the risk of complications.

43% of all hypoglycemia (low blood sugar) episodes and 55% of severe hypoglycemic episodes are reported occurred during sleep. It is practically and technically impossible for HbA1c and glucometer to capture readings while asleep.

Two people can have similar HbA1C values but different average blood glucose levels. This occurs due to individual differences in how your red blood cells and blood sugars bind.

Many faces of 7% HbA1c:
Despite maintaining your HbA1c within target range, your organs are bearing the brunt of high sugar at some (most) point of time.
Note: HbA1c and glucometer also doesn’t indicate anything about sugar fluctuations throughout the day.
What does the report look like?
Experience the difference yourself
The reader gives you two kinds of reports, the actual glucose data for all 14 days and a consolidated report (the graphs, see picture below):
Remember : Continuous glucose monitoring does not permanently replace your glucometer. Both these technologies have their own significance and supplement each other to fine tune your diabetes management.

Frequently Asked Questions

The plan comes with Freestyle Libre Pro Kit, a tool designed for Continuous Blood Glucose Monitoring (CGM) that records your blood sugar levels and trends throughout the day and night for up to 14 days. The kit contains a sensor, sensor applicator, and alcohol wipe.

Yes. The Freestyle Libre Pro allows scanning your sugar levels even through thick clothing and layers.

Absolutely! You can do both. However, do keep in mind that as per Abbott, it is water resistant in up to 1 metre of water and should not be immersed for longer than 30 minutes.

Avoid tight-fitting clothes in the area of sensor application. Be a little careful while moving around to prevent hitting against the walls or doors. It may dislodge the sensor from its place and need reapplication. Reach out to our helpline number {} in case your sensor gets dislodged any time before 14 days.

No. Even if the sensor comes off before the 14 days are up, it cannot be reapplied.

No, there is no way to do this.

No, the exposure may damage the sensor.

It does not connect with a pump but you can use it irrespective of whether you are on pump or injections.

The device has been extensively tested for accuracy and efficacy. However, you could have inaccurate readings for a number of reasons - dehydration, compression lows, use of certain medications such as ascorbic acid (vitamin C), paracetamol, salicylic acid and others.

Abbott recommends the back of the upper arm as the Sensor has currently been tested and approved for use as upper arm wear. Additional studies are underway to approve other insertion sites for the sensor.

Yes. People can wear the sensor while undergoing dialysis too. However, the sensor will need to be removed by a healthcare professional before procedures like CT Scan, MRI, diathermy, etc

This plan offers a Continuous Glucose Monitoring device System kit. It allows you to check Time-in-Range and prevent unexpected life-threatening lows.

Time-in-range

It is the percentage of time a person spends within the target range of their sugar levels. It cannot be identified through any other sugar monitoring tests, including HbA1c, fasting or postprandial. The main motive of diabetes management is to increase this time-in-range.

FreeStyle Libre Pro captures the time-in-range using an ambulatory glucose profile because of which the data is presented in the form of graphs, making it easier to understand the fluctuations.

Management of lows

As per studies, almost half of all episodes and more than half of all severe episodes of hypoglycemia occur at night during sleep. Such episodes are known as nocturnal hypoglycemia.**

At night, when it is practically impossible to monitor your sugar values, continuous glucose monitoring using Freestyle libre pro actively notes down your sugar values even when you're asleep and helps you prevent future nocturnal episodes of hypoglycemia.

  • Real-time monitoring: Records real-time sugar levels every 15 minutes and provides patterns and summaries of day-to-day sugar levels.
  • Gives a detailed sugar picture: Unlike SMBG which only provides a snapshot of your sugar levels, CGM provides a detailed report of sugar spikes, lows and a better picture of your sugar levels throughout your day.
  • No-prick solution: Unlike blood sugar monitors that require pricking of fingers, Freestyle Libre Pro enables you to record sugar levels painlessly. It could give you 288 readings a day without any pricks.*
  • 24-hour tracking: Helps track the sudden lows (hypoglycemia), sudden spikes (hyperglycemia), and change in glucose values throughout the day.
  • Designing an effective lifestyle change plan: Helps you in making your diet and exercise routine efficient by overseeing their impact on your values.
  • Your doctor’s assistant: When your current medicines and lifestyle changes fail to help manage your sugar levels, it gives an insight to your diabetologist and helps them devise a better treatment plan.
Sensor
A tiny sensor wire is inserted under your skin using an automatic –applicator. This collects glucose values continually
Transmitter
A smart transmitter is built in the sensor. This sends real-time readings wirelessly to a receiver
Reader/Receiver
A receiver receives your glucose readings. These readings are translated into dynamic data to form trends and patterns to tailor your health needs

While self-monitoring with glucometer helps measure blood glucose values only at a certain point of time, continuous glucose tracking yields visual data that reveals trends and patterns in blood glucose values. Furthermore, glucometer is limited by the number of fingersticks you are willing to perform per day, whereas, continuous glucose tracking can record up to 288 glucose values per day.
With regards to lab tests such as HbA1c, glucometer gives an overall picture of glycemic control while continuous tracking helps identify three important parameters--glycemic variability, time spent in target glucose range, and risk of extremely low sugar values.

Notably, you may need to use continuous tracking technology occasionally and will definitely require self monitoring of blood glucose at other times.

Remember, all these tracking methods supplement each other to help manage your diabetes optimally and efficiently.

When in doubt, turn to your glucometer

With continuous tracking, you will be able to look at your glucose data on the 14th day only once they are downloaded to the system. Hence, real-time sugar values will still have to be procured with the help of a glucometer.

Pay attention to the low sugar/high sugar symptoms. If you’re not feeling well, use your glucometer to check sugar values at that particular time, and if needed, seek immediate medical attention.

Yes, it is safe for use in people with high blood pressure or kidney disease. People can wear the sensor while undergoing dialysis too. However, the sensor will need to be removed by a healthcare professional before procedures like CT Scan, MRI, diathermy, etc.

The FreeStyle Libre Pro system is clinically proven to be accurate, stable and consistent over 14 days, as per the FreeStyle Libre Pro System Accuracy Trial.* The smart body sensor that comes with the system is calibrated in the factory. So you don’t need to do further calibrations (comparing values with the fingerstick testing) to maintain accuracy. Other continuous tracking systems may require you to perform regular 2 or more calibrations.

As per clinical studies, 14 days of continuous glucose tracking data correlate well with 3 months of continuous tracking data, particularly for mean glucose, time in range, and high sugar measures.** This enables your care team to fine tune your diabetes treatment for the next 90 days. Also, 14-day data are a reliable indicator of the patterns received.
