Disulfiram or Antabuse blocks the action of an enzyme so that alcohol isn't metabolised completely. The build up of aldehyde thus cause reaction which can be severe and one may need emergency management. Monitoring liver function is also necessary.

Naltrexone reduces the reward feeling of alcohol thus reducing its craving. Treatment needs to continue for 3-6 months along with psychotherapy. Disulfiram causes physical reaction to alcohol thereby causing aversion to use of alcohol. There is no evidence as to what is better than other but disulfiram only causes aversion and causes f patient doesn't take it, the aversive effects no longer occur.
