Understanding Sperm Antibody Total
Anti-sperm antibodies (ASA) are abnormal immune proteins that attack sperm cells as if they were harmful invaders. Normally, the immune system defends the body against infections, but in some cases, it wrongly identifies sperm as foreign and produces antibodies against them.
In men, these antibodies may develop after testicular injury, surgery (such as vasectomy reversal), or reproductive tract infections. They can attach to the sperm head, tail, or midpiece, making it harder for sperm to swim, pass through cervical mucus, or fertilize the egg. In women, the immune system may form antibodies in the blood or cervical mucus, impairing sperm function and preventing conception.
The Anti Sperm Antibody test is important in evaluating unexplained infertility, repeated pregnancy failures, or reduced sperm motility despite normal sperm count. It may also be recommended in men after testicular trauma, surgery, or chronic reproductive tract infections, and in women when immune-related infertility is suspected. By detecting anti-sperm antibodies, the test contributes to the overall fertility work-up as part of a comprehensive evaluation and not as a standalone test for treatment decisions.

What does Sperm Antibody Total measure?
The Sperm Antibody Total test measures the presence of antibodies directed against sperm cells. These antibodies may attach to the sperm head, midpiece, or tail, reducing their motility, preventing them from entering cervical mucus, or blocking fertilization with the egg. However, the test does not quantify functional impact or site-specific binding on sperm.
- In men: Anti-sperm antibodies may develop after testicular injury, surgery, or infections, leading to reduced sperm function.
- In women: Antibodies in cervical mucus or blood can attack incoming sperm, preventing successful conception.
A positive result indicates the presence of anti-sperm antibodies, which may contribute to infertility. A negative result means antibodies were not detected, but further fertility evaluation may still be necessary.
