Understanding 17 Alpha Hydroxy Progesterone

The 17 Alpha Hydroxy Progesterone test measures the amount of 17-OHP in the blood. This hormone is a part of the process of making cortisol, an important hormone for stress response, blood sugar regulation and other vital functions. This test helps detect or evaluate congenital adrenal hyperplasia (CAH), an inherited condition that results in decreased cortisol and aldosterone production and increased male sex hormone (androgen) production. The common type of CAH is due to a lack of the enzyme 21-hydroxylase, which helps convert 17-OHP into cortisol. 

The 17 Alpha Hydroxy Progesterone test also helps in monitoring Cortisol Replacement therapy, evaluating infertility and is also useful in evaluating Adrenal and ovarian neoplasm. High levels of 17-OHP in the blood is a marker of enzyme 21-hydroxylase deficiency in Infants with features of Adrenal insufficiency like hypotension, hypoglycemia and hyperkalemia and in infants with ambiguous genitalia. High levels may also be seen in females with clinical evidence of possible androgen excess and showing symptoms such as excess hair growth in areas typical for males, deep voice, increased muscle mass, absence of menstrual periods, or infertility. 

No special preparation is needed for this test. However, an early morning specimen is preferred. The blood sample may need to be collected at a specific time during a woman’s menstrual cycle as different phases have different ranges. Moreover, the test should not be done if an individual is taking steroids, as these can affect the test results.
